A Second Touch support coordination agency is seeking an experienced support coordinator for adults age 21 and over with intellectual disabilities. A Second Touch services Atlantic, Camden, Cumberland, Gloucester, and Burlington County. Responsibilities include initial in-home intake services that assist participants in gaining access to needed programs and State plan services, as well as needed medical, social, educational and other services.

Quarterly face to face monitoring and weekly phone calls is required. The Support Coordinator is responsible for attending the individualized service plan team meetings and maintaining the individualized service plan with the participant, their family, and other team members designated by the participant as needed and annually.

The support coordinator will carry a caseload of 15 to 45 participants.

Candidate will be hired with a caseload of 15 to 20 participants. Support Coordinators will not be required to write plans.

Candidate must be experienced in Irecord; understanding of DDD supports program and CCP. A family oriented, understanding individual and a fast learner who can easily learn specific writing techniques as well as extremely organized, compassionate, time sensitive, and able to work with supervisor as well as independently from home. This position will require coming into the main office once a week. Experience with understanding developmental disabled individuals and organizational skills are a condition of hire.

Experienced in Support Coordination and Irecord system Preferred. If required: A two-day mandated training with the State of New Jersey. Candidate will be paid at $60 per day totaling $120. Training will be paid after working for 90 days. Only individuals who currently hold a bachelor's degree should apply.

Qualifications
Bachelor's Degree or higher AND 1 year of experience in developmental disabilities or care management/coordination in any social services field --and - State, Federal Criminal Background checks and Central Registry check at the time of hire. Successfully complete training are required by the Division of Developmental Disabilities before rendering any service.

Required experience:
Must be exceptionally organized and willing to learn; Knowledge of Developmental Disability services.
